Description
By 2030, the DigiCare Innovation consortium aims to establish a transformative healthcare model based on accessible, equitable, and scalable community care systems. This transformation is driven by the integration of disruptive digital health technologies, the cultivation of dynamic innovation ecosystems, and the promotion of inclusive, patient-centered practices. DigiCare Innovation aims to transform healthcare delivery by focusing on disruptive innovation principles, the objectives of the EIT HEI Initiative, and establish best practices in primary and community care, specifically targeting gaps in oncology, mental health, and chronic disease management. DigiCare Innovation conceptualizes community care environments as dynamic centers for health innovation, utilizing IoT-enabled testbeds and living labs as collaborative platforms for co-creation and empirical validation of solutions. The integration of these testbeds with primary care workflows will optimize chronic disease management, offer mental health support, and improve oncology care. The implementation of gamified tools, predictive analytics, and telemedicine platforms will facilitate proactive and preventative care, enhancing health outcomes while reducing costs.
